Job Summary: The Social Worker will provide direct assistance with the social and emotional needs of patients and families. The Social Worker will follow a plan of care in conjunction with all disciplines. As the Team Lead Social Worker, they will provide leaderrship to other members of the Social Work team. The Team Lead Social Worker will provide clinicial oversight to Hospice Home Care and Solace Social Workers.
Required Education: Masters of Social Work
Preferred Education: None
Required License: MSW; Valid Driver's License required.
Preferred License: LCSW
Required Experience: At least five years' experience providing clinical psychosocial assessments in medical setting
