Blue Ridge Regional Hospital in Spruce Pine, NC is looking for a Medical Social Worker to join the team! This position is part-time, benefited. Anticipated work schedule is Weds, Thurs and every other Friday from 7:30a - 4p (days and hours could vary).
The Medical Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. In partnership with the care team, the social worker is responsible for ensuring the post-acute medical needs and level of care are appropriate. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.
Required Education: Master of Social Work
Applicants without an MSW must enroll in an MSW program within 1 year of hire and complete within 3 years of hire date.
Preferred Certification: Certification in case management highly preferred (ACM-SW or CCM).
Preferred License: State Licensure Preferred (State of North Carolina CMSW, LCSW, or LCSW-A).
Preferred Experience: Experience in acute inpatient hospitals and prior case management roles
