Overview
Job Summary: The Primary RN in Home Health manages an assigned caseload of patients, providing nursing care and clinical case management, taking responsibility to improve patient outcomes and to manage resources and assure cost effective, clinically effective, and coordinated services.Completes comprehensive assessment/physical assessment, develops/modifies plan of care in conjunction with patient/caregiver and physician, provides interventions and discharge planning to these patients to facilitate patient/caregiver engagement for achievement of patient goals, clinical outcomes and positive patient experience. The Primary RN provides the majority of patient's nursing care and communicates handoff information/coordinates with other nurses and members of care team to support patient's care throughout home health and the continuum of care. This nurse demonstrates sensitivity to needs of culturally diverse populations by providing appropriate sound judgement in a professional manner in order to coordinate and supervise patient care.
Required Education: Diploma in Nursing, Associate's Degree in Nursing, or Bachelor's Degree in Nursing
Required License: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. Compact license may apply, licensee should confirm with NCBON; BCLS. Valid Driver's License.
Preferred License: National certification in area
Required Experience: One year of acute experience
Preferred Experience: experience in home health and/or hospice
