Overview

Mission Health is seeking a Discharge Planner to provide discharge planning for the 5 South inpatient behavioral health unit here at Mission Hospital.

 

Dutie include:

 

  • Supports department by providing quality case management and discharge planning services for a specific group of patients hospitalized for treatment of psychiatric or emotional disorders, their families, spouses and significant others.
  • Managing the legal paper work for involuntary commitments and incompetent adults
  • Serving as a member of  the multidisciplinary team to coordinate disposition, schedule appointments, transportation, placement and other regulatory requirements.

 

Required Education: Master degree in social work, counseling, psychology or equivalent bachelors degree in a recognized mental health field


Preferred Education: LCSW(A), LPC(A), BSW or equivalent


Required License: None

Required Experience: One year of experience as social worker/counselor. Two years experience preferred.
