Summary of Job:
The Primary Case Manager is a registered hospice nurse (PCM) who manages and provides care to patients and family/caregivers. The PCM provides nursing care and clinical case management, as well as manages resources and coordinated services with hospice interdisciplinary team. The PCM completes comprehensive assessment/physical assessment, modifies plan of care in conjunction with patient/caregiver and hospice interdisciplinary team, provides interventions and support to these patients and families to manage symptoms, and supports quality of life experience. The PCM provides nursing care to the patient and communicates and coordinates with the full IDG team to support both the patient and the family care throughout hospice experience. The PCM demonstrates sensitivity to the needs of culturally diverse populations by providing appropriate sound judgement in a professional manner in order to coordinate and supervise patient care. The PCM assists the patient and the patient's family with transitions through end of life and understanding of hospice practice and philosophy.
Required Education: Diploma in Nursing, Associate's Degree in Nursing, or Bachelor's Degree in Nursing
Required License or Certifications: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON Certificate: BLS Healthcare Provider.
Required Experience: One year acute care experience
Preferred Experience: Hospice or Home Health experience
