JOB SUMMARY: Responsible and accountable for the daily operations of applicable Care Management services (case management, care coordination, discharge planning, and resource coordination (including collaboration with system utilization management functions) in the inpatient hospital setting. Leads the development and expansion of care management programs in collaboration with the applicable service line leaders. Ensures collaboration occurs and interdisciplinary relationships exist with the caregivers, physicians, and community providers to ensure the seamless transitions for patients back into the community.
REQUIRED EDUCATION: BSN or RN and advanced degree in health related field.
PREFERRED EDUCATION: MSN
REQUIRED LICENSE: Must have and maintain current licensure as Registered Nurse with the NC Board of Nursing (NCBON) or compact state as defined by NCBON. National Case Management certification within I year.
PREFERRED LICENSE: National Case Management Certification.
REQUIRED EXPERIENCE: A minimum of five years nursing experience and two years of relevant Care/Case Management experience with demonstrated leadership skills.
PREFERRED EXPERIENCE: Previous leadership experience in nursing or hospital Care/Case Management.
