Coordinates with the plan of care for community services and resources in a cost-effective and time efficient manner. Serves as a patient advocate and a resource for Ambulatory Care Management Team, multi-disciplinary staff and providers. Collaboratively engages with Care Management staff to ensure appropriate utilization of resources and smooth transition between various community placements and services in the region. Collaborates to achieve optimal patient experience and positive clinical, financial and quality outcomes for assigned patient population across the system.


Required Education: Bachelor's Degree


Preferred Education: Bachelors of Social Work (BSW)


Required License: None


Required Experience: 3 years' experience in health care setting or local social service agency. BSW internship at Mission or other acute care hospital accepted in lieu of required experience.


Preferred Experience: 5 years hospital/healthcare discharge planning experience.


Preferred Certification: Level I: NC Certified Social Worker, Accredited Case Manager (ACM)

Required Certification: Level II Requirement: Maintains nationally recognized certification as NC Certified Social Worker or ACM
