Stroke Program Coordinator at Mission Hospital
The Stroke Program at Mission Hospital is the only Comprehensive Stroke Center certified by The Joint Commission serving the 18 counties of Western North Carolina and beyond.
JOB SUMMARY: The Stroke Coordinator for Mission Hospital is responsible for the development and maintenance of the quality of care delivered to stroke patients and their movement across the continuum of care. Serves as a clinical expert and resource to patient and family, nursing staff, other clinical staff, physicians and the community. Works in collaboration with various medical, clinical and administrative staff to achieve desired outcomes and compliance with evidence based practice, requirements from The Joint Commission (TJC), CMS and other accrediting bodies.
Process Improvement: The Stroke Program Coordinator will be continually evaluating processes and related data across the patients' continuum of care to identify opportunities for improvement, enhahcements, adjustments in order to reach desired outcomes.
Community Outreach: The Coordinator will partner with agencies across the community to provide Stroke education opportunities and to plan and lead related events, to collect data from partner agencies, and to provide feedback as appropriate for the purpose of increaseing the quality of patient outcomes.
Data Analytics: The Coordinator will be responsible for gathering and analyzing data from a variety of sources in order to provide robust reporting, and to provide feedback to agencies and facilities.
REQUIRED EDUCATION: Bachelor's Degree: in Nursing, Emergency Medical Service or other related health sciences
PREFERRED EDUCATION: Master's Degree in Nursing or other related field
REQUIRED LICENSE: NC RN license or NC Paramedic Certification
PREFERRED LICENSE: National Registry for Paramedics or Certified Neuroscience RN or Stroke Certified RN
REQUIRED EXPERIENCE: 3-5 years acute clinical experience
PREFERRED EXPERIENCE: In addition to 3-5 years of acute clinical experience, 1 year of clinical experience specific to stroke care, care management, utilization management or quality improvement experience. Computer skills needed include proficiency in data entry, data analytics, Word, Excel, Access and PowerPoint.
